REGISTRATION FORM

Whether you fill one out a hard copy or an electronic version, use these tips while filling out the registration form:

  • For the Unit Number, type “3322”
  • Fill in the circle for the subscription to Boy’s Life.
  • The Transfer section should be blank unless your son is presently registered with another Pack and is transferring to Pack 3322.
  • Leave the signature area blank for the Unit Leader (or Designee).
  • Please enter a valid e-mail address. E-mail is the main form of communication for the Pack and dens. If you want a second email address included on Scout communications, simply email that to Shawn Petiya at spetiya@hotmail.com

The registration form can be found at the following link: http://www.scouting.org/filestore/pdf/524-406A.pdf. Print the form and fill it out.

Once the registration form is complete, either:

1) Mail the form to the following address:

2) Or, scan the form and e-mail it to Shawn Petiya at spetiya@hotmail.com.

Option 3: Request a Form

To request a registration form send an e-mail to Shawn Petiya at spetiya@hotmail.com. Please provide your name and mailing address. A registration form will be mailed to you. Once the registration form is complete, either:

1) Mail the form to the following address:

REGISTRATION FEES

The registration fees are pro-rated depending on the month the registration is submitted. The fees for 2017-2018 are as follows. The fee includes a subscription to Boy’s Life.

If you are registering with a paper form (not electronically) please provide a check with the proper amount when submitting the registration form. The check can be made out to “Cub Scout Pack 3322.” Send the check to the following address:

MEDICAL FORMS

The Pack is required to have medical forms on file for each participant (Scout, mom, dad, sibling, etc.) attending any Scout event (bicycling, camping, hiking, canoeing, etc.). Please see the following link for the forms.

Print out the forms (both Parts A and B) and fill them out completely. Bring the completed forms to the activity and turn them into Shawn Petiya, the Pack’s Secretary.

FYI – Part A will need to be updated and resubmitted each Scout year. To streamline events in future years, you may want to scan Part A before signing and dating. Then, in future years, if nothing has changed, simply print out the scanned version, sign and date, and submit.

In addition to the medical forms, Pack 3322 is required to have a medical insurance card on file for each family. Please submit a copy of this with your medical forms. Only one insurance card is needed per family.

TALENT SURVEY

As with all Cub Scout organizations, Pack 3322 relies on the assistance of parents to make the Pack fun, educational, and successful. The Pack asks that each family fill out a talent survey to help us identify areas where each family may help the Pack succeed. We ask that you submit a survey with the rest of your registration paperwork.
