The Pack is required to have medical forms on file for each participant (Scout, mom, dad, sibling, etc.) attending any Scout event (bicycling, camping, hiking, canoeing, etc.). Please see the following link for the forms.
Print out the forms (both Parts A and B) and fill them out completely. Bring the completed forms to the activity and turn them into Shawn Petiya, the Pack’s Secretary.
FYI – Part A will need to be updated and resubmitted each Scout year. To streamline events in future years, you may want to scan Part A before signing and dating. Then, in future years, if nothing has changed, simply print out the scanned version, sign and date, and submit.
In addition to the medical forms, Pack 3322 is required to have a medical insurance card on file for each family. Please submit a copy of this with your medical forms. Only one insurance card is needed per family.
